Sunnyside Community Services
Community Outreach Worker responsible for promoting case management services, engaging with community groups, and referring eligible individuals. Requires bilingual skills, organizational abilities, and outreach experience.
The Community Outreach Worker will build awareness of SCS’ Case Management services among local organizations, healthcare providers, social service agencies, and community groups. The right candidate will have strong communication skills, experience with relationship building, and organizational skills to effectively connect older adults with services and coordinate with community partners to promote program services.
When applicable for the job, SCS currently operates on a hybrid work model with three days in the office and two days remote.

Reports to: Case Management Director
Salary: $48,000 – $50,000 annually
