Case Manager, RN (Full‑Time) – Provide skilled nursing assessments, develop and coordinate hospice care plans, document care, counsel families, travel to patient homes. Requires Oregon RN license, ≥1 yr hospice/home‑health experience, driver’s license, CPR certification. This position is responsible for planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians' orders and professional skills, and in coordination with the other members of the interdisciplinary team, the Case Manager develops, coordinates and implements a hospice plan of care that meets each patient's specific needs and is in compliance with Federal and State regulations, reimbursement guidelines, as well as the organization's policies and procedures
